Patient Forms
Before your visit, please take the time to look over and complete the following forms. These documents and their corresponding instructions are provided for your convenience. Upon your first appointment, our team will refer to these forms for patient logs, office sign-ins, and appointment tracking.
Patient Information Form
Save time and fill out paperwork before you arrive. This form contains fields about your basic personal information, contact data, and a patient release.
Workers’ Compensation/PIP Patient Form
This forms asks additional questions for patients with workers’ compensation, personal injury protection, or no-fault insurance claims.
Your First Visit Guide

What To Bring
Your first visit to a new practitioner can be an intimidating experience. To simplify the patient experience, we’ll let you know exactly what to bring to your first appointment. This way, you can start off your spine care treatment as stress free as possible.

- Claim and Attorney Information (Worker’s Comp or Personal Injury)

- Discs and Reports of X-Rays, MRIs, CT Scans, & Other Test Results
Our Process
At Sun Orthopaedic & Spine Care, we follow an organized, structured medical process to treat all our patients. Before you schedule your first appointment, take the time to review our typical process. This way, you can know what to expect before you even walk through the doors.
Information Gathering
We start by gathering information related to your concerns including your medical history.
Physical Examination
We'll conduct a thorough physical examination to determine the possible cause of your pain.
Imaging Studies
We may request certain imaging studies such as X-ray, MRI or CT scan to further evaluate your condition and to determine the best treatment plan.
Conservative Treatments
We always prioritize non-operative conservative treatment options like physical therapy, medication, or chiropractic procedures at the early stage of recovery.
Spine Surgery
If non-surgical treatment does not alleviate your pain, you might be a candidate for surgical treatment. Dr. Sun will discuss the type of the procedure, benefits, risks and the potential outcome with you. A detailed post-operative instruction will be given to you prior to your discharge from the hospital.
